Signer Checklist

• All signers must be present at the time of scheduled appointment and must provide a current government issued photo ID (e.g. Driver's License, CA Senior ID card, US Passport). 

• Signer must be mentally alert and capable of communicating without assistance.

• Except for signatures and notary public acknowledgement or jurat forms, all documents should be filled out prior to the notary appointment.
